If you are a new patient at Kappa Dental Group, please review the documents below in order to prepare in advance for your dental visit. You can print and sign the Patient Registration form and the Acknowledgement of Privacy Practices form and bring them with you to the office.
Please read the Notice of Privacy Practices form carefully and keep it for your own records. This notice describes how health information about you can be used and disclosed, and how you can access this information.
Tip: If you choose to complete the documents when you are at the office, please plan to arrive 15 minutes prior to your scheduled appointment.
